As parent/guardian of this child(ren) we understand that we have have a sacred responsibilty to see to their religious education. This responsibility can only be fulfilled if we work together with the clergy and staff of the Sunday School. We therefore promise to make a conscious effort to see to it that these children attend Sunday School regularly and to work to reinforce their lessons at home. 

I give persmission for my child(ren) to participate fully in the Sunday School program at First Congregational Church. In case of an emergency, I undertstand that every effort will be made to contact the parents of the child(ren). In the event I cannot be reached, I give permission for the Sunday School staff to secure proper and necessary treatment for my child(ren).
